Joining the CaFHS team in Port Lincoln, you will provide families with information and support to access early childhood intervention services within their local area. You will also liaise with other agencies regarding early intervention coordination for children 0-8 years with, or at risk of, developmental delay/disability. Assessing immediate family needs and establishing appropriate referral pathways in collaboration with each family, you will encourage them to take an active role in decision making and action on behalf of their child. This will include assisting families to access and understand the contents of assessment and other reports regarding their child, and promoting the Early Childhood Intervention Consultant program to other agencies. Operating with a person and family centred care approach, you will work collaboratively with relevant agencies/committees in order to address gaps in service provision. You will have proven experience in working with children with developmental delay and communicating with a range of children, families and communities with diverse cultures and value systems. With sound knowledge of available Early Childhood Intervention Services and the National Disability Insurance Scheme, you will bring the ability to work in partnership with a range of disciplines and community agencies. You will work well both independently and as a member of a team to determine priorities, organise workloads and meet deadlines.
