A Health & Wellness Promoter is required to create opportunities for Southern Cross Care residents to participate in physical activity via individual and group exercise programs.
Reporting to the Residential Services Manager, you'll work as part of a multidisciplinary Early Intervention team, to improve mobility and quality of life, while identifying and developing recovery pathways for those at risk of frailty and decline.
Developing and designing tailored exercise programs that suit resident needs and goals, your work will form part of a long term plan with residents to improve their abilities and quality of life, and will include screenings, fitness appraisal and instruction. Along the way, you'll review activities to ensure all residents are able engage in activities best suited to their abilities.
Your responsibilities will also extend to gym coordination, ensuring our Health & Wellness Centre has suitable resources for programs and is maintained in a clean and safe manner. You'll also support our team of volunteers and students assisting the centre and its activities, and maintain resident program records for annual reviews. In addition, you'll liaise with management to ensure continuous improvements are implemented and evaluated.
Demonstrating competencies in goal setting in a person centred care environment, you'll have experience in selecting appropriate exercise programs for older individuals including those with impairments and frailty. You'll possess excellent project management, planning, time management and leadership skills, and be able to collaborate in case management situations with others to make effective decisions and problem solve.
A Certificate IV, Diploma of Fitness or higher is essential for this position, and accreditation with Physical Activity Australia, Fitness Australia or Exercise & Sports Science Australia is required.
