About Monash Health

Monash Health is an impressive organisation of seven public hospitals, 21 community health sites, five aged care facilities, university affiliated international research and tertiary education. We provide healthcare for the entire lifespan from pre-birth to palliative care and all stages in between that supports the delivery of clinical are.

In a new initiative between Monash Health and the Department of Health, in April 2018 we will be opening a new Transition Support Unit (TSU) to support clients with a dual disability to transition from hospital to community care. With several permanent roles available, we're now looking for energetic, committed nurses with a keen interest in dual disability to help drive our Model of Care in this new 10 bed unit.

About the TSU

The TSU is a community based service, which will be a transition point for consumers between an acute setting and community, or community and the more intensive supported environment of the TSU. Clients eligible for the TSU are also likely to be eligible for the National Disability Insurance Scheme (NDIS).

Operating on a model of least restrictive care, including reduction in the role of chemical restraint in addressing behaviours of concern, we will incorporate a co-design approach in the delivery of activities of daily living. Each of our clients will work in partnership with the occupational therapist and peer worker in designing relevant activities (ie. groups) consistent with their behavioural support plan.

About you

You have a passion for psychiatric nursing. You may already have expertise in Intellectual Disability although this is not mandatory. More important is your genuine interest in supporting clients in this specialist area as we will work collaboratively with you to develop and maintain any existing skills gaps you may have.

You are a recovery focused nursing professional with insight into the recovery model of care. You have a keen interest in dual disability and a focus on recovery with individual care planning. To be considered for this role, you need

  • Current AHPRA registration
  • Working with Children's Check
  • Proven relevant clinical experience
  • Relevant qualifications
