About this opportunity

The National Disability Insurance Scheme (NDIS) is a new service that will provide support, care and access to services and technology for Australians with disabilities as well as their families and carers.

As a Support Coordinator, you will coordinate a variety of services for participants to support them to live a fulfilling and meaningful life in the community. In addition, you will build the capacity of participant to self-manage their supports and link them in with a wide range of mainstream services and natural supports to provide additional support and care. In supporting participants, you will work inclusively and build great relationships with families and carers.

What do you need?

  • Minimum Certificate 4 in Mental Health, Disability, Aged Care, Home and Community Care or other related fields
  • Excellent interpersonal and communication skills
  • Demonstrated experience/knowledge in working with people with mental health issues and their carers to build strong, long lasting partnerships in the community
  • A current valid Driver's Licence and a registered vehicle (including insurance) and be willing to travel to surrounding suburbs
  • Satisfactory Police Records Check and Working with Children Check, prior to commencement
  • All experience within the mental health, AOD or dual disability sector will be highly regarded (from volunteering, to placement, to 1:1 work, all experience will be valued highly)
  • Membership with the Australian Association of Social Workers, Australian Community Workers Association or equivalent or AHPRA will be considered favourably
